Beef Liver for Postpartum Recovery: What the Research Says

Liver has moved from something your grandmother served on a Tuesday to something people take in capsules and powders.
 
In postpartum circles it comes up constantly, usually with a lot of enthusiasm and not much detail.
It is worth separating two things: what the nutrient data actually shows, and what gets claimed on the internet.

This article covers what is in freeze-dried beef liver, what the research says about those nutrients in the postpartum period, and, just as importantly, what has not been studied. It also covers vitamin A, which is the most important safety consideration and the reason liver is not suitable for everyone.

All reference values here are the Australian ones, set by the National Health and Medical Research Council.

What is actually in beef liver

Beef liver is one of the most nutrient-dense foods in the human diet. That is not marketing language, it is simply what the composition data shows.

Why freeze-dried changes the maths

Fresh liver is roughly 70 percent water. Freeze-drying removes almost all of it while leaving the nutrients behind, so what remains is around three and a half times more concentrated by weight than the fresh original.

That means a gram of freeze-dried liver powder is not a gram of liver. It is closer to three and a half grams of fresh liver, compressed.

These are calculated from the fresh values using a 3.4 times concentration factor, not directly measured, so treat them as close estimates rather than precise figures. Actual values vary with the animal, the season and the drying process.

Two things follow from this, and they pull in opposite directions.

The useful part. A small daily amount of powder delivers a meaningful nutrient contribution without anyone having to cook and eat liver. That is the entire appeal.

The part that requires attention. Concentration applies to everything, including the nutrients that have upper limits. Vitamin A and copper both become more concentrated at exactly the same rate as iron and B12 do. A more nutrient-dense product is not automatically a better one. It is a reason to look at the label rather than a reason to skip looking.

Why the postpartum period is different

Pregnancy and birth draw heavily on iron stores. Blood volume expands, the growing baby takes what it needs, and birth itself involves blood loss.

The Australian picture is clearer than most people expect. The South Australian Perinatal Practice Guideline on perinatal anaemia reports that during a pilot of the Australian Red Cross Lifeblood Maternity Toolkit, 60 to 70 percent of women screened were iron deficient. The same guideline defines postpartum anaemia as a haemoglobin below 100 g/L, and treats a serum ferritin below 30 µg/L as iron deficiency requiring treatment.

What that means day to day has been studied directly. The South Australian guideline links perinatal iron deficiency to 2.5 times higher odds of antenatal depression, along with increased postpartum depression, fatigue, impaired cognition and altered maternal-infant bonding.

A 2022 systematic review and meta-analysis in BMC Public Health pooled 27 studies and found that iron deficient or anaemic women were 1.66 times more likely to experience symptoms of depression than iron-replete women. Pooled data from three randomised controlled trials showed a statistically significant reduction in fatigue scores in women who received iron replenishment.

Australian anaesthetists writing in Anaesthesia in 2023 argued the point directly, noting that iron deficiency affects between 50 and 70 percent of women across pregnancy and the postpartum period, and that universal iron deficiency screening should be built into peripartum care.

This is the part worth sitting with. The exhaustion many women describe in the months after birth is real, it is measurable, and in a meaningful proportion of cases it has a nutritional component that responds to being corrected.

It is also not the only cause of postpartum fatigue, which is why the section below on getting tested matters more than anything else in this article.

A note on the Australian iron target

The Australian RDI for iron during lactation is 9 mg per day, which surprises people. It is lower than the 18 mg for menstruating women and much lower than the 27 mg recommended in pregnancy.

The reason is that many women are not menstruating while breastfeeding, and very little iron passes into breast milk. The lactation figure reflects reduced ongoing losses, not reduced need for recovery.

That distinction matters. The 9 mg RDI describes maintenance for a woman whose stores are intact. It says nothing about rebuilding stores that pregnancy and birth have already run down. If your ferritin is low, maintenance is not the goal and the RDI is not the relevant number. Your doctor's is.

Haem iron and why the form matters

Iron in food comes in two forms. Haem iron comes from animal tissue. Non-haem iron comes from plants and from most iron supplements, which use iron salts such as ferrous sulfate.

They are absorbed by different pathways, and not equally.

A 2022 isotope study in Nutrients measured iron absorption from matched meals in healthy women. Adjusted non-haem iron absorption was 21.7 percent from a beef protein meal compared with 4.2 percent from a texturised fava bean meal. A 2025 review in Nutrients describes haem iron being absorbed through active transport catalysed by haem oxygenase in the intestinal enterocyte, a mechanism distinct from non-haem iron uptake and less affected by dietary inhibitors such as phytates, polyphenols and calcium.

Tolerability differs too. A 2024 Cochrane review of 33 randomised trials in 4,558 postpartum women found that oral iron tablets probably result in a large increase in constipation compared with intravenous iron. A separate 2024 meta-analysis in the European Journal of Nutrition found participants receiving haem iron had a 38 percent relative risk reduction in total side effects compared with non-haem iron.

Here is the honest qualification on that last finding, because it is the kind of statistic that gets quoted without its footnote. The authors graded the certainty of that evidence as very low, found no statistically significant difference between haem and non-haem iron for iron status indicators in most population subgroups, and concluded that the results need further investigation through high-quality clinical trials.

So: haem iron is better absorbed and appears to be gentler on the gut. Whether that translates into better outcomes for correcting a diagnosed deficiency is not settled.

Vitamin B12

Beef liver is among the richest known sources of vitamin B12. The Australian RDI during lactation is 2.8 µg per day, and B12 is one of the nutrients where maternal status transfers directly to the breastfed infant.

A widely cited review in Advances in Nutrition estimated that an infant fed by a B12-deficient mother obtains only about 16 percent of the recommended daily intake from her milk. That was the lowest figure of any B vitamin assessed, well below thiamin at 60 percent, vitamin B6 at 80 percent and choline at 56 percent.

The clinical picture is consistent. A 2023 systematic review in Nutrients analysing 292 published cases of infant B12 deficiency found that maternal B12 status, exclusive breastfeeding and a maternal diet low in B12 predicted infant B12 levels, and that mean age at first symptoms was four months.

The risk is concentrated in mothers eating little or no animal food, or with absorption issues. MotherSafe, the NSW Health medicines in pregnancy and breastfeeding service, advises that vegetarian and vegan mothers, and those with malabsorption, should be supplemented with vitamin B12 while pregnant and breastfeeding. If that describes you, this is a conversation for your GP rather than a supplement decision, because B12 deficiency in an exclusively breastfed infant is serious and testing is straightforward.

Choline, the one most Australian mothers miss

Choline gets far less attention than iron or B12, and the Australian intake gap is larger than either.

The NHMRC Adequate Intake for choline is 440 mg per day in pregnancy and 550 mg per day during lactation. Australian women are not close.

A 2019 analysis in Nutrients built a choline database for the AUSNUT 2011-13 food composition data and applied it to the Australian National Nutrition and Physical Activity Survey. The finding: fewer than 10 percent of the Australian population, across every age and gender group, achieved the Adequate Intake for choline. Eggs were the top-ranked source.

Pregnancy-specific Australian data agrees. A 2022 study in Nutrients of 103 pregnant Australian women found a median intake of 362 mg per day, with only 39 percent above the NHMRC Adequate Intake. The Barwon Infant Study, published in the European Journal of Nutrition in 2023, found a mean of 372 mg per day with just 23 percent of women meeting the target. A 2025 summary in Proceedings of the Nutrition Society put the range across Australian studies of pregnant and lactating women at 1 to 40 percent meeting their Adequate Intake.

Why it matters: a 2022 systematic review and meta-analysis in Advances in Nutrition found low maternal choline intake was associated with higher odds of neural tube defects, and that higher maternal intake during the second half of pregnancy and the early postnatal period was safe and likely to favourably affect child neurocognition across domains including memory, attention and visuospatial learning.

Fresh liver provides around 333 mg of choline per 100g, or roughly 11 mg per gram of freeze-dried powder. Eggs are the other standout source, and are the more practical everyday option for most people.

Copper, and why serving size is small

Liver is an extraordinary source of copper: roughly 9.8 mg per 100g fresh, against an Australian Adequate Intake of 1.5 mg per day during lactation.

The NHMRC Upper Level of Intake for copper in adults is 10 mg per day. A 100g serve of fresh liver sits almost exactly at that ceiling on its own.

Copper is essential, and it is part of why liver's profile hangs together as a package rather than a list, because copper is required for iron metabolism. But it is also the clearest illustration of why liver supplements come in small serves and why "more concentrated" is not a synonym for "better."

The NHMRC notes these upper levels do not apply to people with Wilson's disease or other copper handling disorders, who need different management entirely.

The vitamin A conversation

This is the most important section in this article, and it is the reason liver is not for everyone.

Beef liver contains preformed vitamin A, or retinol. This is not the same as beta-carotene from carrots and sweet potato, which the body converts as needed and which does not accumulate to harmful levels. Preformed vitamin A accumulates.

The Upper Level of 3,000 µg per day applies equally to non-pregnant women, pregnancy and lactation.

Against that ceiling, liver is in a category of its own. Raw beef liver contains around 4,968 µg RE per 100g. Healthdirect, the Australian government health service, lists liver at 10,726 µg per 100g, well ahead of the next-highest food on its list. The variation reflects the animal, the cut and whether it is raw or cooked. Either figure is several times the daily upper limit in a single serve.

The NHMRC is explicit about why the limit exists: "the critical adverse event used for women of childbearing age was teratogenicity." In plain terms, birth defects. Healthdirect puts it as "there is a possibility of birth differences (defects) if you are pregnant and have too much vitamin A."

This is why liver is not recommended during pregnancy. If you are pregnant or trying to conceive, this is the whole answer, and no amount of nutrient density changes it.

For breastfeeding, the same 3,000 µg upper limit applies, but the recommended intake is higher at 1,100 µg per day, so there is more headroom than in pregnancy.

The practical takeaways:

  1. Check the vitamin A per serve on any liver supplement label. If it is not disclosed, that is worth asking about.

  2. Do not stack sources. Liver plus a multivitamin containing retinol plus cod liver oil adds up quickly.

  3. If you take a prescription retinoid such as isotretinoin or acitretin, liver is not appropriate for you. Speak to your prescriber.

  4. The NHMRC notes that people with high alcohol intake, existing liver disease, high blood lipids or severe protein malnutrition are particularly susceptible to excess preformed vitamin A.

What the research has not shown

An article promising what the research says owes you the gaps as well as the findings.

No randomised controlled trial has tested a freeze-dried beef liver supplement for postpartum recovery outcomes. Not for fatigue, not for mood, not for iron status. Everything above is evidence about nutrients and about foods, not evidence about a product. Anyone telling you a liver supplement is clinically proven to fix postpartum exhaustion is describing a study that does not exist.

Wholefood supplements deliver nutrition, not therapeutic doses. If you have diagnosed iron deficiency anaemia, the South Australian perinatal guideline recommends 100 to 200 mg of elemental iron daily for three months for stable women with a haemoglobin below 100 g/L, with intravenous iron increasingly used first line. No wholefood product reaches those amounts, and none is intended to.

Fatigue after birth has many causes. Iron deficiency is one. Thyroid dysfunction, B12 deficiency, sleep deprivation, postnatal depression and simple recovery from a major physiological event are others, and they frequently overlap.

If you are exhausted in a way that is not shifting, the highest-value thing you can do is not to choose a supplement. It is to ask your GP for bloods, including a full blood count, ferritin, B12 and thyroid function. Knowing what is actually going on changes what helps.

If you are unsure whether any of these apply to you, ask your GP, midwife or pharmacist before starting.

How to think about it

Beef liver is a genuinely nutrient-dense food with a well-documented composition, and several of the nutrients it concentrates are the same ones commonly depleted after birth. That is a reasonable, evidence-consistent basis for including it in a postpartum diet, whether as food or in a considered supplement form.

What it is not is a treatment. It sits alongside eating enough, resting when the opportunity exists, and getting tested when something feels wrong. It does not replace any of them.

If you cannot face liver as food, which is most people, a small daily amount of freeze-dried powder is a reasonable way to get the nutrient profile without the Tuesday dinner. Look for liver from grass-fed cattle, check the vitamin A per serve, and check that the source is disclosed.


Frequently asked questions

Is beef liver safe while breastfeeding?

Beef liver is generally considered suitable while breastfeeding. The key limit is vitamin A. The NHMRC sets an Upper Level of 3,000 µg per day of preformed vitamin A for lactating women. Check the vitamin A per serve of anything you take, and avoid taking multiple retinol-containing supplements at once. If you have any doubt, check with your GP or midwife.

How much beef liver is too much?

The Australian Upper Level of Intake for preformed vitamin A is 3,000 µg per day. Raw beef liver contains around 4,968 µg RE per 100g, and Healthdirect lists liver at up to 10,726 µg per 100g. Copper is the second limit: liver holds roughly 9.8 mg per 100g against an adult Upper Level of 10 mg per day. This is why liver supplement serves are measured in grams rather than tens of grams.

Is freeze-dried liver stronger than fresh liver?

Per gram, yes. Fresh liver is about 70 percent water, and freeze-drying removes it, leaving a powder roughly three and a half times more concentrated by weight. That applies to every nutrient equally, including vitamin A and copper, which have upper limits. More concentrated is not automatically better. It means the serving size is smaller and the label matters more.

Is haem iron from liver better absorbed than iron tablets?

It is better absorbed. An isotope study measured 21.7 percent adjusted non-haem iron absorption from a beef protein meal versus 4.2 percent from a plant protein meal, and haem iron uses a separate absorption pathway that is less affected by dietary inhibitors. Whether that translates into better clinical outcomes than iron supplementation is not established, and the meta-analysis comparing them graded its evidence as very low certainty.

Will beef liver fix my postpartum fatigue?

No study has tested that. Beef liver concentrates several nutrients commonly depleted after birth, and there is good evidence that correcting iron deficiency reduces fatigue. But postpartum fatigue has many causes, and no trial has assessed a liver supplement against any postpartum outcome. If your exhaustion is persistent, ask your GP for a full blood count, ferritin, B12 and thyroid function.

How common is iron deficiency after birth in Australia?
